Rehab designed for active people. Built for the long game.

PHYSICAL THERAPY

You don't want to be standardized. You want to be developed. From your very first session, you'll get a personalized plan built around your goals — not a generic protocol designed for everyone else.

Why Continuum Physical Therapy?

How is Continuum different then traditional PT?

1

Most traditional PT models were built around pain relief, passive treatment, and getting patients through visits. At Continuum, every session is built around development — not standardization.

We:

  • Take the time to listen to your full history and understand what you're trying to get back to

  • Find the root issue, not just treat the symptom because sometimes your hip pain is actually causing your back problems

  • Build real strength and capacity alongside rehab so you come out performing better than before

  • Teach you how to understand your body and progress on your own — so the problem doesn't keep coming back

Who’s it for?

2

  • You're an active person dealing with pain who doesn't want to be told to stop doing what you love

  • You've been through traditional PT and never fully got back — or the problem keeps coming back

  • You want to return to your sport, your training, or everyday movement with a real plan that builds you back stronger

  • You want to understand your body and keep progressing on your own long after treatment ends

What should I expect?

3

  1. Evaluation + plan — a full assessment of your history, goals, and what you're trying to get back to

  2. In-person sessions — 1:1, hands-on, and built around building competency and capacity

  3. App-based support — individualized programming, weekly check-ins, and 24/7 messaging so the work continues between sessions

The PT → Training bridge

Physical therapy is our foundation — and for many people, it's just the beginning. If your goal is to keep building capacity after rehab, we'll transition you into personal training with a plan that makes sense. Same team, same roof. No starting over, no losing momentum.

“I had worked with three other physical therapists. My condition either worsened or showed no improvement. I was beginning to lose hope. From my very first session, I could tell things would be different. He took the time to listen carefully to my history, asked thoughtful questions, and explained his approach in a way that made me feel both confident and understood.”

— Vitali S.

“I had completely stopped running. Dr. Nick correctly identified weaknesses and developed a structured program unique to my goals. After four months, my mobility is better than pre-injury. Most importantly, I learned how to progress my workouts independently to prevent injury in the future.”

— Kelli C.

“After dealing with plantar fasciitis for almost 18 months, I thought it was just something I'd have to live with. Addressing that not only helped eliminate my pain, but also significantly improved my mobility and overall performance — bringing me back to a level I never thought I'd reach again.”

— Dylan B.

“In being treated for numerous injuries over the years and having 4 surgeries, I've had a lot of PT. The spectrum of quality is huge. Nick is one of the best I've seen.”

— Kevin S. (orthopedist)
